How to Enroll

1. Click here to access the online enrollment system. Note: It may take up to 5 days to access the enrollment tool, however, your enrollment period begins on your date of hire.

2. Simply follow the instructions. The site will lead you through the steps to enroll.
- Be sure to review your selections and save them.
- Once your enrollment is complete, keep a copy of your Confirmation Statement for your records.

Time sensitive! Enroll and provide dependent documentation within 31 days of hire.

Eligibility

If you are a full time employee of CMC, working 30 hours or more per week, you are eligible for benefits.

Note: Employees in Hawaii are eligible for HMSA benefits if they work 20 hours or more per week for 4 consecutive weeks.

Dependent Eligibility

You can cover the following dependents:

• Your legal spouse, including a same-gender spouse, or your common-law spouse*
• Domestic partner*
• Your children, who include:
- Natural children
- Stepchildren
- Legally adopted children
- Foster children
- Children of your common-law spouse
- Children for whom you have legal guardianship

You can cover your children:
• Up to age 26
• Up to any age for physically or mentally disabled children, as long as you provide proof of disability—contact Employee Services to learn more.

*Where required

You may enroll eligible dependents for CMC benefits if you submit the required documentation by your enrollment deadline.
